NAREA Conferences & Initiatives MEDICAL RELEASE, PHOTO RELEASE, & LIABILITY WAIVER FORM In consideration of allowing me, the undersigned, to participate in any way in the above referenced related events and/or activities (collectively referred to as the “Event”), I knowingly and voluntarily enter into this Participant Waiver and Release of Liability Agreement. I acknowledge the risks associated with traveling to and from, and participating in, the Event; my own physical condition; situations beyond the control of the Event organizers; and other undefined, unforeseeable, and unknown risks and dangers. I assume all related risks and responsibilities, known or unknown, of my participation. I attest that I am over 18 years of age unless otherwise indicated, am free from illnesses, injuries, and defects that could interfere with safe participation, and accept that participation is entirely voluntary. I release, indemnify, and hold harmless the North American Reggio Emilia Alliance, Inspired Practices in Early Education, Inc., event directors, volunteers, organizers, host facilities, and related Releasees from claims arising from my participation. I consent to first aid and other medical treatment in the event of injury or illness and am financially responsible for associated costs. I grant permission for the Event and Releasees to use photographs, images, video, motion pictures, recordings, or other depictions of me or my participation for legitimate purposes in perpetuity without compensation. I have read this Agreement carefully, understand its terms and conditions, agree to be bound by them, and acknowledge that I am giving up substantial legal rights by signing this Agreement freely and voluntarily.
